Do You Need a Referral for Physiotherapy in Ontario?

A Complete Guide for Hamilton Residents

If you’re dealing with pain, an injury, or recovering after pregnancy or surgery, you may be wondering: Do you need a doctor’s referral for physiotherapy in Ontario?

The short answer is usually no—but there are important exceptions, especially when it comes to insurance coverage, OHIP-funded physiotherapy, and workplace or motor vehicle injuries.

This guide explains exactly when you do and don’t need a referral, so you can confidently book physiotherapy in Hamilton without delays.

No Referral Needed for Private Physiotherapy Clinics

Physiotherapists in Ontario are primary healthcare providers. This means you can book an appointment directly with a physiotherapist without first visiting your family doctor or a walk-in clinic.

At Flexwell Physiotherapy in Hamilton, you can book directly for:

  • Back and neck pain

  • Sports injuries

  • Pelvic floor physiotherapy

  • Prenatal and postpartum physiotherapy

  • Muscle and joint pain

  • Dry needling

  • Post-surgical rehabilitation (in many cases)

No referral is required to start treatment.

When Might You Need a Referral?

Although a referral is not legally required for physiotherapy in Ontario, there are certain situations where one may be necessary for insurance coverage or funding purposes.

1. Insurance Companies and Extended Health Benefits

Do Insurance Plans Require a Referral?

Some extended health insurance plans require a physician’s referral in order to reimburse physiotherapy services.

Important points to know:

  • This is an insurance requirement, not an Ontario legal requirement.

  • Many insurance providers no longer require referrals.

  • Requirements vary between providers such as Sun Life, Manulife, Canada Life, and Green Shield.

What Should Patients Do?

Before your first appointment:

  • Review your insurance policy.

  • Contact your benefits provider.

  • Ask specifically whether a physician’s referral is required for physiotherapy reimbursement.

At Flexwell Physiotherapy, our team can help you understand what documentation may be needed for your insurance claim.

2. OHIP-Funded Physiotherapy

Does OHIP Cover Physiotherapy in Ontario?

Yes, but only under specific circumstances, and a referral is required.

You may qualify for OHIP-funded physiotherapy if you are:

  • 65 years of age or older

  • Under 19 years of age

  • Recently discharged from a hospital following:

    • Surgery

    • A fracture

    • An acute injury or illness

  • Receiving Ontario Works (OW) or Ontario Disability Support Program (ODSP) benefits

  • Living in a long-term care home

Important Considerations

  • A referral from a physician or nurse practitioner is required.

  • Services are only available through designated OHIP-funded clinics.

  • There may be wait times and limitations on the number of visits.

  • Patients cannot choose any clinic they wish to attend.

Most private clinics, including Flexwell Physiotherapy, are not OHIP-funded. However, private care often provides faster access, individualized treatment, and flexible scheduling.

3. Motor Vehicle Accident (MVA) Claims

Do You Need a Referral After a Car Accident?

In most cases, a referral is not required to begin physiotherapy after a motor vehicle accident.

However:

  • Treatment is typically funded through auto insurance.

  • Certain forms and approvals may be required.

  • Insurers may request medical documentation.

At Flexwell Physiotherapy, we help patients navigate:

  • Motor vehicle accident paperwork

  • Insurance communication

  • Treatment planning for whiplash, back injuries, and soft tissue injuries

4. Workplace Injuries and WSIB Claims

Do You Need a Referral for WSIB Physiotherapy?

A doctor’s referral is not always required, but:

  • The injury must be reported to WSIB.

  • Approval is generally required before treatment begins.

  • Medical documentation may be necessary.

Our clinic can assist with coordinating care and guiding patients through the WSIB process.

Can a Physiotherapist Assess You Without a Referral?

Yes.

Ontario physiotherapists are trained to:

  • Perform comprehensive assessments

  • Diagnose musculoskeletal and pelvic health conditions

  • Identify potential red flags

  • Refer patients to a physician or specialist when necessary

Direct access to physiotherapy allows patients to receive care more quickly, reduce wait times, and begin recovery sooner.

Why Patients in Hamilton Choose Direct Access Physiotherapy

Many patients prefer direct access physiotherapy because it offers:

  • Faster appointment availability

  • No need to wait for a family doctor or walk-in clinic visit

  • Evidence-based assessments

  • Personalized treatment plans

  • One-on-one care

At Flexwell Physiotherapy, we focus on patient education, hands-on treatment, and long-term recovery outcomes.

When Should You See a Doctor First?

Although physiotherapy is often the appropriate first step, you should consult a physician before treatment if you experience:

  • Unexplained weight loss

  • Fever or signs of infection

  • Severe trauma

  • Progressive neurological symptoms

  • A suspected fracture or serious medical condition

Your physiotherapist will also refer you to a physician if any concerning symptoms are identified during your assessment.
